A Member’s health card (or ID#) and an ETIQA Letter of Guarantee (LOG) are required to avail of healthcare benefits through No-Cash-Out, from ETIQA’s Medical Information Center (MIC). An LOG is required for consults. A doctor’s request with diagnosis is required to get an LOG for lab & diagnostic tests.

Major hospitals & clinics include: Makati Medical Center, St Luke's Medical Center (Quezon City & Global City), Cardinal Santos Medical Center, Asian Hospital & Medical Center, The Medical City,  Healthway Clinics, Hi-Precision Diagnostics, MyHealth Clinics, and QualiMed Clinics.

Plan Members can avail of any inpatient, outpatient, or emergency service as a private patient for reimbursement at any hospital or clinic worldwide, and reimburse at the accredited rate: the rate ETIQA would have paid the doctor, hospital, or clinic had they been accredited. 

An ETIQA Claim form and an Official Receipt (OR) are required to avail of healthcare benefits through Reimbursement, from ETIQA’s Claims department. A doctor’s request with diagnosis is required to reimburse for lab & diagnostic tests; a doctor’s prescription is required to reimburse for equipment & supplies. 

Doctor Anywhere provides telemedicine services through their app. Setup of member's Etiqa account is required. Learn more at Go2.purplecow.ph/DA.

Pre-Existing Conditions (PEC) refers to medical conditions that existed before healthcare coverage, whether or not life-threatening. Purple Cow provides 100% PEC coverage, up to MBL. 

Congenital Conditions refers to medical conditions that existed before birth. PurpleCow provides 100% Congenital Conditions coverage, up to MBL. 

Special Procedures/Modalities refers to alternative medical procedures that are not invasive, or do not involve surgery. PurpleCow provides 100% Special Procedures coverage, up to MBL.

Annual Benefit Limit

ANNUAL BENEFIT LIMIT is a single aggregate limit for all disabilities per year (sickness or accident), whereas MAXIMUM BENEFIT LIMIT is per disability per year. ABL can be applied on a per-rank basis. ABL rates are slightly cheaper than MBL rates.

INDEPENDENT OF PHILHEALTH allows non-PhilHealth members to be discharged without filing for PhilHealth, or paying for PhilHealth's share of the bill.

Note: Etiqa will not enroll member in PhilHealth. IPH can be applied on a per-individual basis.

Exclusions

  1. Expenses that should be taken care of by government programs such as PhilHealth; 
  2. Services rendered, or supplies provided, free of charge; 
  3. Incremental costs from taking a higher room accommodation than Member’s R&B limit; 
  4. Non-recommended confinement; convalescent/domiciliary/custodial care; rest cures; 
  5. Medico-Legal Fees, including the cost of Medical Certificates; 
  6. Procedures that are not medically necessary, or that are used for screening purposes only; medical equipment purchased in support of medical treatment, including but not limited to: nebulizer/suction/CPAP machines, TENS/thermo-scan/BP devices, and all test kit supplies; (covered under optional BRO) 
  7. Dental-related cases, except for (optional) Dental benefits, and cases of accidental injury; Vision-related cases (i.e., errors of refraction), except for Lasik to treat for legal blindness; (covered under optional BRO)
  8. Annual Physical Examinations, except for (optional) APE benefits; Executive Check-Ups; (covered under optional BRO)
  9. Take-home or prescribed (Rx) medicines; Vaccines, except for anti-venom/rabies/tetanus; (covered under optional BRO)

MEDICAL AVAILMENT

A Letter of Guarantee (LOG) is required to avail of inpatient or outpatient services in accredited medical facilities. Lala (PC) lala@purplecow.ph can assist. 

1) For outpatient LOG (consultations/lab tests/OP surgery), you can request LOG 3 ways:
